Nevins Adult Day Health Center provides top quality respite daytime care to adults 18 years and over who have medical, cognitive and social impairments. Adults attending the Center come from many environments such as Private Homes, Group Homes, Adult Foster Care, Assisted and Supportive Living Programs.

The Center is ideal for families and caregivers looking for assistance with the care of their loved ones, by offering a relaxed atmosphere for their family member to socialize with others and be involved in daily activities.

Health Care Services

Our nursing staff coordinates thorough medical plans for our clients, working with their physicians, therapists and community agencies. A care plan for each client is developed and is reviewed quarterly by all disciplines.

Additional services offered include:

  • Medication teaching and management, blood pressure monitoring, weight management, diabetic monitoring
  • Occupational and Physical therapies
  • Pharmacy
  • Podiatry
  • Medical transportation
  • Nutritious breakfast, lunch and snack

Social Services

  • Individual and family counseling
  • Quarterly review of each client's care plan
  • When needed, we can help families find services for home health care and housing
  • Discharge planning and assistance with nursing home placement is provided, if needed
  • A transportation plan is developed at the time of admission to the program

Activities

Our daily group activities are creative, emotionally satisfying, individualized, success oriented and ever-changing in a calm, well-structured environment. An on-site barber and hairdressing services are available.
